It's the SGI 030-1145-001 board, with audio and video I/O connectors but - I guess - without the A/D video converters (see photo) Connector is a 62x2 (A01 to A49 - A52 to A62 and B01 to B49 - B52 to B62) with slot at A50/A51 and B50/B51 (see photo) Plastic cover lacks of 3 of 4 locking tabs
